For your convenience, you may register your child with AACPP from this page in one of two ways:

Download and print the forms on the left side below, and complete them.

Then either:

forward them to us by mail or fax, and our staff will call you to schedule appointments.
or
call 610-372-9222 and follow the prompts for an appointment with our intake staff and return the forms to them in person at our office between 8 am and 6 pm.
Please include a copy of their insurance card (front and back please),

2. Complete the forms on the right online, save to your computer, and email them to us at intake@aacpp.com. Our intake staff will call you to schedule an appointment.
